  • Once you have completed this process, Liens NC will present you with a page which has a square QR Code. This is the page you will print out and include a copy of with your building permit application.
• Fill Permit Form - Required for ALL Single Family Dwelling Applications
• Trade Affidavit Form - Required for all Sub-Contractors listed on Permit Application. 
• Elevation Certificate (if applicable)
• CAMA Permit (Waterfront property only)
• Appendix F Form (if applicable) - only used if Owner is acting as General Contractor and is Unlicensed.
• Property Taxes must be paid in full at time of Application.

• Final Septic Permit / Health Department Release (If Applicable)
• Culvert & Ditch Certification (If Applicable)
• Final Elevation Certificate (If Applicable)
• Lot Certification of Elevation (If Applicable)
• Termite Treatment Affidavit 
• Energy Forms - Energy Efficiency, Air Sealing, Duct Leakage Test
• Appendix R1 - Energy Certificate to be permanently mounted - Energy NC 2018 Code
• Appendix R3 Visual Inspection - Energy NC 2018 Code
• Appendix R3C Duct Testing- Energy NC 2018 Code
• Appendix R3B  Air Sealing Testing Option- Energy NC 2018 Code
